Northern Disability Linkages

Linkages provides assistance for people with disabilities who have complex and high support needs living at home. The service applies a "Case Management" approach to meeting client/carer needs, and has some funding available to purchase extra services available if needed.

 

Aim

  • To offer people whose primary disability is of a physical nature, and their carers, choices and control about their care options.

  • To provide levels of support and service beyond what is already available.

  • To work with individuals, families and service providers to encourage coordination, flexibility and quality service provision.

  • To assist with transition between various types of care.

  • To provide a service which is centred on the needs of the individuals and their quest for an acceptable quality of life and independence within their own home and local community.

  • To ensure clients rights are upheld.

Who is Eligible

People with complex care needs associated with a disability including:

  • Children from birth to eighteen years who live with their families, and adults up to 70, who live alone, or with a carer who may be ageing, unwell or in need of support. People who require help to co-ordinate the services they need.

  • People who want to continue living at home who reside within the municipality of:

    • Banyule

    • Whittlesea

    • Darebin

    • Nillumbik

Referrals

Referrals will be taken from people with a disability, their carers, friends and service providers. People are accepted onto the Linkages service on the basis of their need. Client/carer permission is essential for the transfer of confidential information. Acceptance into the program depends on suitable eligibility and available places.

 

How can we help?

Northern Disability Linkages strives to be flexible and creative in responding to each person's needs. A holistic assessment and care plan will recognise and be sensitive to the specific and cultural needs of the client and carer. A fee for service applies and is based on income.

  • Specific Equipment

  • Household Tasks

  • Activities

  • Meals

  • Respite for carers
