Community Health Nursing

The Community Health Nursing Service provides a prompt and accessible service for adolescents and adults including people with a chronic illness and their carers, to maintain and promote their health and well-being.  The service provides diagnosis, assessment, management and health promotion and service may be delivered individually, in groups and may be centre or home-based. Services include:

Assessment – comprehensive health assessment for prevention and management of chronic disease
Community education including schools and community organizations
Carer education and support
Health promotion activities
Education, including aids/equipment
Liaison with other service providers
Secondary consultation
Advocacy

Program Includes:

• Healthworks program – providing assessment, education, exercise program and support groups for people with heart disease and/or diabetes.

• "Don't Miss a Beat" - exercise and support program for people with chronic heart failure.

• Womens Cancer Support Group
